We are getting ready for our curbside meals over Spring Break and want to be prepared. There will be paper forms distributed at each of the meal pick-up sites for meal scheduling. A digital version of the same form can be found here. Only one form needs to be filled out per family. If you have already filled out the paper copy there is no need to fill out the digital version. If you fill out the digital version just let the staff at the meal pick-up site know that you have filled out the digital version.
